The presentation aims to give HCPs an understanding of the different types of diabetes, the importance to treatment of diet/lifestyle and activity, and the referral criteria for the National Diabetes Prevention Programme  (DDP). If you would like a recording of the presentation narrated by a diabetes specialist please contact the LMNS on hny.lms@nhs.net and we will send you a copy.

Referrals to the DPP

The DPP is an evidence-based behaviour change programme that supports people identified as being at high risk of developing type 2 diabetes. It is proven to be effective in reducing the risk of developing T2D. Over 9 months, individuals receive personalised support with healthy eating and lifestyle, increasing physical activity and managing their weight. There is no cost to participants.

Post pregnancy HCPs can log onto the Healthier You Humber and North Yorkshire website: Diabetes Prevention Programme In Humber & North Yorkshire (preventing-diabetes.co.uk) then click on the Health Professional section and click on the Humber and North Yorkshire section where they will find a link to the form to send to the address below.

MSW Gestational Diabetes Referral Co-ordinators have been employed in each of the HNY Trusts to facilitate referrals to the DPP.
